Montecito Physical Therapist Ordered to Cease Practice
LOS ANGELES – A Montecito Physical Therapist has had her license revoked following her criminal convictions on charges of driving under the influence.
The Physical Therapy Board of California revoked the license of Sara Evelyn Hazen, PT, by default after Hazen failed to file a Notice of Defense in response to the Board’s filing of an Accusation.
In May 2011, after a multi-vehicle traffic collision, Hazen was arrested and charged with driving under the influence of blood alcohol content (BAC) of .08% or greater and in September 2011 pled no contest. In December 2011, Hazen was arrested and charged with driving under the influence of blood alcohol content (BAC) of .08% or greater after colliding with a rock and in April 2012 pled no contest.
Her license revocation becomes effective July 8, 2013.
